The vaccine is for clostridium perfringens Types C&D aka "overeating disease". It happens mostly to healthy well-fed animals. In kids it usually happens when they pretend they're little pigs and drink an overload of milk. The easiest way to explain it is that the bacteria that normally live in the goat's stomach reproduce at an above average rate when the goat gets a lot to eat and then create a toxin level that can actually kill the goat. (That is a very simple way of putting it and you should read about it to get a more detailed description of it.) This vaccine is to prevent that. The "T" part is tetanus. Very important anyway but especially is you have or have had horses around. Hope that helps a little!
